* We can entertain NEMT risks up to 20% of Fleet (80% of Fleet must be Ambulance).

■ Limited States: AL, CA, CT, DC, DE, FL, IN, KY, MI, MD, MO, MS, PA, SC, TX

■ Ineligible States: AK, GA, HI, LA, MA, NV, NJ, NY, WV 

Submission Requirements:

• Supplemental Application
• Acord Applications: 125 and 137/State
• Vehicle Schedule
• Driver Schedule 
• Driver MVRs
• Currently Valued Loss Runs (Minimum 5 Years unless in operation for less)
• Historical Vehicle Count & Premium for the Current and 4 Prior Policy Years
• Forward any information available concerning Safety, Loss Control or Driver Training Programs 
• Cameras and or Driver Telematics in place (if any)
• Target Premium(s)
